New Patient
For all new patients, please read the welcome letter outlining expectations for your first visit. Patients must complete the New Patient packet including New patient intake form, Records release form and the HIPAA form. Sending this information to us before your appointment is preferred and will significantly decrease your check in and waiting time. If time doesn’t allow you to get the packet to us prior to your appointment, please have it completed before your arrival.
